Telford & Wrekin Council launches innovative Adult Social Care Portal
Telford & Wrekin Council is pleased to announce the launch of the new Adult Social Care Portal, an online platform designed to improve the way Adult Social Care services are accessed and managed. The portal is set to provide comprehensive support and streamline communication for both residents and professionals alongside the existing ways to contact Adult Social Care.
The Adult Social Care Portal is available to residents in Telford and Wrekin who wish to make a referral, or who are already receiving support from Adult Social Care. It offers a centralised online hub where users can easily make a referral into Adult Social Care, with future developments also including the ability to contribute towards their assessments; receive copies of assessments and plans; and communicate with their allocated professional. The Adult Social Care Portal was developed in co-production with people who have lived experience, ensuring it truly meets the needs of those who use it.
This initiative represents another significant step forward in enhancing the accessibility and efficiency of Adult Social Care services, ensuring that residents can navigate their care journey with greater ease, choice and confidence.
In addition to empowering people, the portal serves as a vital tool for professionals, enabling them to make a professional referral into Adult Social Care via the portal.
The rollout of the Adult Social Care Portal will take place in phases over the coming months. People will have the choice as to whether they sign up to use the portal or continue to follow Adult Social Care traditional routes of contact. Telford & Wrekin Council is committed to supporting its residents with a range of options to make contact with services and ensuring that everyone has access to the resources and information they need.
